9/2/2023 EnchantaVaughn Approved 1.Behavior: QMHP-C greeted JaCarei upon arrival to the home setting. Upon arrival to the home setting, JaCarei appeared to be in a positive mood evident by his facial expressions such as a smile on the face. QMHP-C engaged him in a conversation to determine how his day was at school. He verbalized to QMHP-C that he had a bad day because he was trying to fight this boy in class because he took his seat. 2. Behavior: JaCarei participated in the session with normal behaviors as he did not need any redirection. At times throughout the session, he needed some redirection because he wanted to talk over QMHP-C when discussing his behaviors. 1.Intervention: QMHP-C utilized role play to illustrate how easy it is to not let others upset him. QMHP-C engaged JaCarei in a therapeutic activity to assist him with expressing his anger and frustrations towards those in the home and community setting. QMHP-C reiterated on using learned conflict resolution and problem-solving skills to handle disagreements with others. 2. Intervention: QMHP-C read a handout on consideration and allowed JaCarei to discuss what he gathered from the handout with the reoccurring causes of disagreements between peers/adults. QMHP-C demonstrated techniques for becoming an active listener to JaCarei. 1.Response: JaCarei appeared to be receptive to QMHP-Cs’ interventions and services that was provided to him during the session. JaCarei was able to discuss with QMHP-C his anger and frustrations, along with express hurt feelings toward his actions. 2.Response: Through play JaCarei engaged in play and demonstrated conflict resolutions. JaCarei expressed to QMHP-C that at times he continues to not utilize the coping skills that have been provided to him by QMHP-C throughout the sessions, but when he calms down he remembers those coping skills such as counting to ten and deep breathing.
